CASE REPORT

A 64-years old woman presented with a nodule in the outer upper quadrant of her left breast. She had noticed the breast lump four months earlier. On palpation, the mass was freely movable and elastic in consistency. There was no evidence of axillary lymphadenopathy. The remainder physical examination was unremarkable. There was no family history of breast cancer. Results of laboratory examinations were within normal values. Eight months earlier she had undergone left total nephrectomy in this Hospital for excision of a renal clear cell carcinoma, pathologic stage pT2N0MX . She had not received any additional treatment after surgical intervention. The patient had no additional contributory medical history. She underwent surgical excision of the lesion by outer upper quadrantectomy. Frozen section examination excluded cancer and the diagnosis of a benign soft tissue unclassified neoplasm was rendered. Axillary lymphadenectomy was not performed. Following definitive histologic diagnosis she was clinically re-examined and no sign of the tuberous sclerosis complex could be demonstrated. Pathologic specimens of the left total nephrectomy sample were reviewed and the diagnosis confirmed. Thoracic and abdominal CT-scans were unremarkable.

METHODS

Hematoxylin and eosin-stained sections from formalin-fixed, paraffin-embedded blocks were examined for histologic evaluation. Additional 5µ-thick sections were stained with Masson’s trichrome stain and Weighert method (for elastic fibers), and immunostained for smooth muscle actin (SMA)(Dako Corporation, Carpinteria, CA, dilution 1:30), desmin (Dako, prediluted), Factor VIII-related antigen (Dako, prediluted), HMB-45 (Dako, diluted 1:50) using the avidin-biotin complex (ABC) immunoperoxidase technique (Dako, LSAB2).
