Psyllium powder, a component in bulk laxatives, is known to induce allergic reactions when inhaled by occupationally exposed persons. We report on two nurses working in a geriatric residence suffering occupational asthma as a result of inhalation of psyllium dust which they administered to patients. Skin prick tests with psyllium extract (1:10 w/v) were positive in both patients. One of them showed high levels of specific IgE againts psyllium (CAP System). We carried out exposure tests with psyllium dust. An immediate and a dual bronchial reaction were seen (the patient with isolated immediate response had taken systemic corticosteroids after immediate reaction). One patient showed a positive methacholine bronchial test. In the other patient this test was negative, this patient had been one year without symptoms and without exposure to psyllium dust. These new cases of occupational asthma from laxative drugs containing psyllium confirm the necessity to change the powder form for another that does not form inhalable particles (e.g. granules). In the case of patients exposed to psyllium dust (occupatinally or through drug intake) we must rule out psyllium hypersensitivity.
