We describe the case of a 63 year-old man who developed a verrucous carcinoma on the site of chronic ulcers of the left leg as the product of artefactual paraffinoma. Staging showed nodular lesions of the right lung consistent with squamous cell metastases on fine needle biopsy. Cutaneous verrucous carcinoma is a low grade squamous cell neoplasia with slow growth and high infiltrative power which can sometimes develope in the site of a varicous ulcer, decubitus ulcer, traumatic ulcer, scars from burns or chronic radiodermatitis. The avarage duration of ulcers varies from 20-34 years. Chronic ulcer is a risk factor for squamous cell carcinoma with metastatic potential.
