ABSTRACT

We present the longitudinal study, clinical and electrophysiological study of 101 patients with spinal stroke and 175 patients with vascular chronic ischemic myelopathy. All cases were confirmed by neuroimagistical examinations: CT-myelography, MRI and selective spinal angiography. Electrophysiological examination included needle electromyography, sensitive and motor electroneurography, F-wave study, Hoffman reflex, bulbo-cavernosus reflex, somatosensory and motor-evoked potentials. 30 healthy subjects were considered as a group of references.

In spinal stroke patients three vascular syndromes were considered: anterior spinal artery syndrome, syndrome of complete transversal lesion and posterior spinal arteries (artery) syndrome.

The patients with chronic ischemic myelopathy were divided in several groups according to dominant clinical syndrome: spastic, spastic - atrophic, atrophic, amyotrophic lateral sclerosis syndrome, poliomielic and siringomielic syndromes.

Clinical and electrophysiological findings were assessed in each case together with etiological factors and the level of ischemic spinal lesion.

Electrophysiological abnormalities were founded in 100% of cases. Based on statistical analysis of the results, electrodiagnostic criteria were elaborated for the discrimination of each syndrome of spinal stroke and chronic ischemic myelopathy. In addition to this data were founded that ischemic damage of spinal cord tissue causes the morphofunctional reorganization of motor units and segmentar neuro-muscular apparatus. Moreover, as a result of neuronal plasticity at this level of nervous system news programs of motor function are established.

The general conclusion of this work is that multimodal electrophysiological investigation as a consciously extension of clinical examination is very important in the positive and differential diagnosis of ischemic disturbances of spinal cord blood circulation. An electrophysiological examination is also useful for the prognosis of ischemic vascular myelopathy and for the control of treatment efficacy.

Practical recommendations of the study consist from clinical - electrophysiological - neuroimagistical algorithm of evaluation of vascular ischemic myelopathy.
