Case Report
fiogf49gjkf0d Multiparous woman, 72 y.o., referring a history of 2 month genital bleeding. Gynecologic examination showed a friable mass of the cervix and uterine cavity. A Pap smear and biopsy samples were taken from both sites.
Mujer multípara de 72 años que consulta por sangrado genital de 2 meses de evolución. En la exploración ginecológica se observó una masa friable afectando a cérvix y endometrio. Se le realizó una triple toma ginecológica y toma de biopsias de la masa en ambas localizaciones.

DIAGNOSIS

Pathology
fiogf49gjkf0d Cytology
Cytology, in exocervical and endocervical smears, shows atrophic changes with an inflammatory and hemorrhagic background. There are also some atypical, mainly mononucleated, cells and some multinucleated ones. They have an ill-defined cytoplasm and large nuclei, with irregular shapes, and a big sized nucleolus.
Biopsies
Biopsies from both localizations (cervix and endometrium) revealed similar findings, and they were consisting of a diffuse proliferation of lymphoid cells with marked desmoplasia, a polymorphic inflammatory background with frequents eosinophils and intermingled Reed-Sternberg Cells (RSC)
Ancillary techniques
By immunohistochemical study, RSC were positive with CD30, and CD15, and negative with CD45, CD20, EMA, CD3, CD45Ro, and ALK. The accompaning inflammatory component showed positive staining with B and T markers. Epstein Barr virus LMP-1 protein was positive.
Molecular biology studies demonstrated B policlonality.
The diagnosis was: Classical Hodgkin disease (OMS, 2001) of the cervix and endometrium
Citología
La citología en las tomas exo y endocervical, muestra un frotis atrófico con fondo inflamatorio y hemorrágico, y con presencia de algunas células atípicas, la mayoría mononucleadas y otras multinucleadas, con citoplasma mal definido y núcleo grande , de contorno irregular, con nucléolo de gran tamaño.
Biopsias
La biopsia de ambas localizaciones presentan hallazgos similares, y están constituídas por una proliferación difusa de estirpe linfoide con marcada desmoplasia, fondo inflamatorio polimorfo, frecuentes eosinófilos y Células de Reed-Stemberg (CRS) entremezcladas.
Técnicas especiales
Mediante estudio inmunohistoquímico, las CRS muestran positividad para CD30 y CD15 así como negatividad para CD45, CD20, EMA, CD3, CD45Ro y ALK. El componente inflamatorio acompañante presenta tinción positiva para marcadores B y T. La Proteína LMP-1 de Virus de Epstein Barr es positiva.
Mediante técnicas de Biología Molecular se ha demostrado policlonalidad B.
Se realizó el diagnóstico de afectación cervical y endometrial por Linfoma de Hodgking Clásico (OMS, 2001).

Evolution
fiogf49gjkf0d Multiple big sized lymphadenopathies were detected in the abdominal CT scan, and a 6 x 5 cm mass adjacent to cervix and uterus body, without infiltration of urinary bladder or rectum. Six months later, after 4 quemotheray cycles (ABVD), the patient has no residual tumor.
El TAC abdomino-pélvico reveló múltiples ganglios linfáticos de gran tamaño pélvicos y una masa de 6x5 cms adyacente a cérvix y útero, sin infiltración de vejiga o recto. A los 7 meses del diagnóstico, tras 4 ciclos de quimioterapia (ABVD), la paciente no presenta tumor residual.

I don’t think the diagnosis can be made in cytology, at least with the submitted images.
Congratulations for such an interesting case.
Indiagnosticable en mi criterio desde el punto de vista citológico, al menos con las imagenes remitidas.

Diagnosis can be made in the histological slides. The microscopic tissue environment must make us think of it. In cytology, it is difficult.
El diagnóstico se puede hacer en cortes histológicos. Se debe pensar por el microambiente del tejido. En citología difícil.

Muy difícil diagnóstico, más aún cuando se observan celulas epiteliales con marcados cambios reactivos simulando un carcinoma epidermoide poco diferenciado o pleomórfico con infiltrado linfoide y eosinófilo. Es probable que el antecedente clínico de linfoma Hodgkin ganglionar nos guíe en el diagnóstico. Tuvo antecedentes?. Felicitaciones por el caso y gracias por presentarlo, voy a estudiarlo nuevamente. ¿REALMENTE HICIERON ESTE DIAGNOSTICO EN CITOLOGIA?
ES MUY DIFICIL. ESTOY DE ACUERDO QUE PUEDE DECIRSE QUE ES MALIGNO PERO NO CREO PODER DIAGNOSTICAR ESTA ENFERMEDAD
