Resumen de la historia clínica:

Twenty-three year old male Caucasian patient, athlete. Painless nodule in the right thyroid lobe which had appeared recently. No alterations in a thorough workup except for a 5cm solid nodule in the right thyroid lobe on CT scan. A right hemithyroidectomy was performed; grossly the nodule occupied almost the entire lobe and it was firm, grey with ill defined yellow areas, 5x4x3cm and focally extending to the surgical margin.

Completion of the thyroidectomy was performed later; the patient is well.

Differential Diagnosis:

• Poorly differentiated carcinoma (Insular)
• Neuroendocrine carcinoma
• Metastatic carcinoma- primary unknown

Diagnosis:

Carcinoma neuro-endócrino – metastático ou primitivo (a evolução – 5,5 anos – aponta para primitivo)
